Community Services

We offer a range of services and programs which allow individuals to age at home with dignity and continuing independence.  Our services and programs address the needs of seniors and/or adults with disabilities at various points along the continuum of care from the most robust to frail adults and adults with Alzheimer’s disease and/or dementia.   Please click on a link below to learn more about an individual service or program:

The Adult Day Services program provides supervised social, recreational and therapeutic programming for seniors and/or adults with disabilities who are unable to attend regular community activities because of physical and/or cognitive impairment, including dementia.

Area served is east of the Humber River; west of Spadina/Bathurst; south of Lawrence; north of St. Clair.

The Case Management Team provides assistance and supportive counselling to seniors who have identified the need for assistance in accessing supports and services while living in the community.. In-home and in-office assessments are provided to plan and coordinate services to meet the needs of the individual.

Area served is east of the Humber River; west of Spadina/Bathurst; south of Lawrence; north of St. Clair.

Community programming is offered at 7 locations within the service boundaries. Activities include social dining, outings and special events. Culturally driven programming is also offered.

Eglinton Hill Centre is a bright, airy facility, newly built in 2009.  It has a highly visible storefront location (Eglinton Hill Room), an office and reception area, a Computer Lab, and a large dividable programming space with a kitchen and pantry (Marco De Luca Room) and a large community room.

Therapeutic falls prevention is a physiotherapy-based group exercise program, led by registered kinesiologists, to address problem areas. General exercise programs are led by trained staff and/or exercise facilitators and are somewhat faster paced.

Falls Prevention referrals are made through the Manager of Health and Wellness at 416-248-2050 x 5238. Exercise referrals are made through the Community Development Team at 416-248-2050 x 5261.

Home Help assists seniors in their home with routine household activities and other services to enable their independent living. Respite Care offers in-home, short-term relief for family and caregivers, allowing them free time for other activities.

The Meals on Wheels program provides home delivery of nutritious meals to seniors and/or adults with disabilities. Meals are delivered by staff or volunteers who provide regular social contact and also check on clients’ safety and well-being. We deliver hot meals, frozen meals, and fruit and vegetable boxes.

The Assisted Living program provides 24-hour on-site personal care and support services for seniors and/or adults with disabilities living in designated Toronto Community Housing buildings. Both short- and long-term supports are offered.

Locations served are 1901 Weston Road, 855 Roselawn Ave, 130 Vaughan Road, 1775 Eglinton Ave West, and 55 Outlook Ave.

Referrals for residency are made through Housing Connections at 416-981-6111. Referrals for Assisted Living are made through the Case Management Team at 416-248-2050.

The Transportation program provides personalized door-to-door assisted rides to frail seniors and/or adults with disabilities who are unable to use public transportation and do not qualify for Wheel-Trans. Our program is part of Toronto Ride, a collaborative partnership of thirteen not-for-profit community support service agencies providing the service.  Group Shopping trips from specific apartment buildings, within our service boundaries are also available.
