
What is the Community Support
For Seniors Program?
The Community Support for Seniors Program
(case management) helps older adults reach
their recovery goals and live successfully in
their community of choice. The program
provides intensive community-based
support for older women and men.
Who can benefit?
Two groups of people are eligible:
- Men and women who are 65 years of age
or older with a serious mental illness;
- Men and women who are 60 years of age or
older with a serious mental illness and physical
or mental health issues related to aging.
What are the major benefits?
The benefits are substantial and include:
- Assistance managing mental
health symptoms
- Assistance learning strategies for
independent living
- Assistance accessing suitable community
resources including housing, income support,
medical, legal, social/recreational, vocational
and educational services
- Assistance addressing physical health issues
- A focus on recovery
