Health Access Program


A partnership between Parkdale Queen West Community Health Centre and Reconnect Community Health Services

The Health Access Program is a time-limited case management program focused on system navigation and care coordination. Its goal is to support newly discharged clients to transition back into the community and to reduce hospital readmissions. Clients will receive timely access to community supports, care coordination between new and existing services, and discharge planning once service integration has been secured. Referrals are accepted from hospitals, primary care and community agencies.

Eligibility Criteria

 

Clients must be:

  • Pending discharge from hospital or be newly discharged from hospital (one month or less)
  • Have an acute or persistent health condition
  • Be in need of services, supports or resources to live safely in the community
  • in need of identification and connection to appropriate community services to reduce hospital readmission and/or advocacy, navigation and care coordination support of health and social services
  • Be a resident of South Parkdale
  • Not already connected to case management services
