Community Mental Health Treatment Teams:
Our community mental health treatment teams are the Reconnect Assertive Community Treatment Team (ACTT), South Toronto LINK Team, and West Toronto LINK Team. These collaborative teams of mental health professionals offer intensive support to individuals with complex mental illness, addictions and co-morbid issues. Some of the service options include nursing, behaviour therapies, case management, psychiatry and a Nurse Practitioner.
Area served is the Toronto Central Local Health Integration Network (TC LHIN), boundaries which includes Victoria Park Avenue to the East; Islington Avenue to the West; the Lake to the South and Eglinton Avenue to the North.

Forensic Case Management Services
Forensic Case Management is a partnership with the Centre for Addiction and Mental Health (CAMH) Law and Mental Health Program, that supports individuals, 16 years and older, with serious mental illness who have been found Not Criminally Responsible (NCR) and are thus under the jurisdiction of the Ontario Review Board (ORB).This program services the TC LHIN. Referrals come directly from CAMH

Supporting Clients Currently in Housing
Reconnect provides support to individuals currently in housing in partnership with Supportive Housing in Peel (SHIP), Canadian Mental Health Association (CMHA) Toronto, and Mainstay. These programs serve the Toronto Central and Central West LHINs. Housing with CMHA and Mainstay is accessed by completing an application through Access Point.
Addictions Supportive Housing (ASH)
Reconnect provides support to older adults (55+ years) with problematic substance use and complex care needs. In partnership with Mainstay Housing, this program delivers wrap around supports to tenants including long-term individualized case management, eviction prevention support, harm reduction-centered addictions counseling and social/recreational activities. All referrals are received through Access Point

Community Outreach Programs in Addictions (COPA)
Provides assessment, treatment and care to older adults (55+ years) who are struggling with addictions in the community. The area served by COPA is the TC LHIN. Referrals are made by completing the COPA referral form and faxing to 416-248-6557 .
Treat at Home
Provides mental health support service to older adults (55+ years) in partnership with CMHA Peel and Canes Community Care. This program services the Central West LHIN. Referrals are made by downloading a referral form from CANES Community Care and faxing to 416-743-7654.
Mental Health and Addictions Supports in the Central West Local Health Integration Network (CW LHIN)
This service supports individuals 16 and over struggling with mental illness and addiction (concurrent disorders), in the catchment area from Etobicoke Rexdale to Dufferin County.
Central West Concurrent Disorders Specialized Support Services (CW CD 3S) – working closely with area hospitals, CW CD 3S offers short-term, specialized interventions for people living with mental illness and addictions (concurrent disorders), to successfully transition clients into the community. The program services The Central West LHIN. Referrals are made by completing the CWCD referral form and faxing to 416-248-6557.
Mental Health and Addiction Services to the Homeless
Streets to Homes
In partnership with the City of Toronto, Reconnect offers support to recently housed individuals 16 years and over, identified by the City of Toronto as a part of their Housing First Homeless Initiative. The program serves clients within the Greater Toronto area. Streets to Homes receives referrals directly from The City of Toronto and is based on the Housing First strategy for addressing homelessness in Toronto.
Short Term Crisis Bed
The Mental Health and Justice Short-Term Crisis Bed program provides short stay residential support in the community to individuals who are living with serious mental illness, are involved with the judicial system and are in crisis. This program services the Greater Toronto Area (GTA). Referrals are made by phone to the CMHA registry by calling 416-248-4174.
Prevention Case Management
The Mental Health and Justice Prevention Case Management provides case management services to individuals 16 years or older that have a mental illness (concurrent/dual diagnosis or cognitive disability included) to reduce the risk of involvement with the judicial system. The Prevention program serves the TC LHIN and North Etobicoke. Referrals are made by completing the MHJ referral form and faxing to 416-248-6557 .
Crisis Outreach Services for Seniors (COSS)
An on-call mobile crisis intervention and outreach service for older adults (65+ years) with addictions issues or mental illness. The COSS team provides direct in-person contact with clients within 24 to 48 hours in a community setting. Short-term crisis response is available 365 days a year from 9am to 5pm. Interventions include crisis counselling, short-term intensive case management, harm reduction and concurrent disorder services, mental health and addictions support, health assessment and care as need by a Nurse Practitioner and referral to primary care. COSS is a partnership between WoodGreen Community Services, Reconnect Community Health Services, LOFT Community Services, Haven Toronto and South Riverdale Community Health Centre.
