Referrals-Health Services

Reconnect provides a range of clinical services that support clients 16 years and older who are dealing with complex mental illness and addictions. Most of our programs are accessed through The ACCESS POINT. However, some are not. For specific direction on how to access the program you are interested in, please go review the programs below.

  • Case Management

    Referrals are made directly through Access Point or by completing the Access Point Referral Form and faxing it through to 416-499-9716. For more information please call 416-248-2050.

  • COMMUNITY MENTAL HEALTH TREATMENT TEAMS

    Includes the Reconnect Assertive Community Treatment Team (ACTT), South Toronto Link Team, and West Toronto Link Team. Referrals are made directly through Access Point or by completing the Access Point Referral Form and faxing it through to 416-499-9716.

  • JUSTICE MENTAL HEALTH SERVICES

    Referrals for Forensic Case Management Services come directly from CAMH.

  • SUPPORTING CLIENTS CURRENTLY IN HOUSING

    All referrals for Supporting Clients Currently in Housing and Addictions Supportive Housing (ASH) are received through Access Point.

  • MENTAL HEALTH AND ADDICTIONS SUPPORTS FOR OLDER ADULTS

    Referrals to Community Outreach Programs in Addictions (COPA) are made by completing the COPA referral form and faxing to 416-248-6557. Referrals for Treat at Home are made by downloading a referral form from CANES Community Care and faxing to 416-743-7654.

  • MENTAL HEALTH AND ADDICTIONS SUPPORTS IN THE CENTRAL WEST LOCAL HEALTH INTEGRATION NETWORK (CW LHIN)

    Referrals for the Central West Concurrent Disorders Specialized Support Services (CW CD 3S) are made by completing the CWCD referral form and faxing to 416-248-6557.

  • MENTAL HEALTH AND ADDICTION FOLLOW UP SUPPORTS TO THE HOMELESS

    Referrals for Streets to Homes are received directly from The City of Toronto and is based on the Housing First strategy for addressing homelessness in Toronto.

  • CRISIS SERVICES

    Referrals to the Short Term Crisis Bed are made by phone to the CMHA registry by calling 416-248-4174.
    Referrals to Prevention Case Management are made by completing the MHJ referral form and faxing to 416-248-6557. Referrals for Crisis Outreach Services for Seniors (COSS) can be made by calling the COSS team at 416-217-2077.
