
The Transitional Case Management (TCM)
Program provides support to individuals who
have a serious mental illness and are involved
with the criminal justice system. The goals of
the TCM program are to facilitate a successful
transition from hospital to community and
prevent future criminal justice involvement
for individuals who are under the jurisdiction
of The Ontario Review Board (ORB).
The program is a partnership among Reconnect,
Canadian Mental Health Association
Toronto Branch
and the Centre for Addiction and Mental
Health
(CAMH) Law and Mental Health Program.
Who can benefit?
Men and women, 16 and over, who are
under the jurisdiction of the ORB and
patients of the CAMH Law and
Mental Health Program are eligible.
