Our Services

Reconnect offers a wide range of community-based services for seniors, caregivers and people living with mental health and addictions concerns. Whether your healthcare goals are to feel better, live independently, stabilize your health or learn coping skills, we have services and programs to help you achieve them.

Our services fall into three main categories:

Community Support Services for Seniors

Services and programs which allow seniors to age at home with dignity and continuing independence.

Mental Health and Addictions Programs

Help to stabilize your health, connect you to the social and healthcare systems, and learn to manage your mental health.

Family and Caregiver Support Services

FAME supports family members and caregivers who have a loved one experiencing a mental health concern.

Adult Day Program

55 years+ or Adults with Disabilities

More Information

The Adult Day Services program provides supervised social, recreational, and therapeutic programming for seniors and adults with disabilities who are unable to attend regular community activities because of physical and/or cognitive impairment, including dementia.


Assertive Community Treatment Teams (ACTT)

16 years+

The Assertive Community Treatment Teams (ACTT) are teams of mental health professionals who work together to provide intensive support to individuals with urgent and complex mental, physical, and emotional needs.


Assisted Living Services

55 years+ or Adults with Disabilities

More Information

The Assisted Living program provides on-site personal care and support services for seniors and adults with disabilities. Many of our services are provided in designated Toronto Community Housing buildings. Both short- and long-term supports are offered.


Case Management

15 years+

More Information

Case management offers one-to-one supports by a case manager who will help you to identify and work towards your healthcare and life goals.


Central West Concurrent Disorders Specialized Support Services

16 years+

Central West Concurrent Disorders Specialized Support Services (CW CD 3S) supports individuals 16 and over struggling with mental illness and addiction (concurrent disorders), living in the area from Etobicoke Rexdale to Dufferin County, in the Central West LHIN.


Community Outreach Programs in Addictions (COPA)

The COPA program helps adults 55 years and over who struggle with addictions that impact their daily lives.


Crisis Outreach for Seniors (COSS)

65 years+

More Information

Crisis Outreach Services for Seniors (COSS) is an on-call mobile crisis service for older adults (65+ years) with addictions issues or mental illness. The COSS team will come in-person to meet with the senior in crisis within 24-48 hours either in their home or in their local community. Short-term crisis response is available 365 days a year from 9am to 5pm.


FAME Family and Caregiver Support Services

Children and Youth aged 7-17 years and Adults 18 years+

Reconnect's FAME Family and Caregiver Support Services offer a range of supports to individuals who are acting as a caregiver for someone who is experiencing a mental health concern. Services include drop-in peer support groups, one-on-one counselling, and specialized programming for children and youth.


Health & Wellness Centre for Seniors

55 years+ or Adults with Disabilities

More Information

The Reconnect Health and Wellness Centre for Seniors offers social, recreational, and exercise-based programming for older adults and seniors. Groups are offered both virtually and in-person.


Home Help and Respite Care

55 years+ or Adults with Disabilities

More Information

The Home Help and Respite Care programs help older adults to stay in their homes longer by providing personal care and/or light housekeeping.


Meals on Wheels

55 years+ or Adults with Disabilities

More Information

The Meals on Wheels program provides home delivery of nutritious meals to seniors and adults with disabilities. Meals are delivered by staff or volunteers who check on clients’ safety and well-being. We deliver hot meals, including Ukrainian and Italian meals, frozen meals, and fruit and vegetable boxes.


Mental Health and Justice Prevention Case Management

16 years+

Mental Health and Justice Prevention Case Management provides case management to individuals 16 years or older that have a mental illness (concurrent/dual diagnosis or cognitive disability included) and current involvement in the criminal justice system or who are at immediate risk of involvement with the judicial system.


Primary Care Inter-Professional Team

13 years+

The Primary Care Inter-professional Team is a partnership of health care organizations supporting the family doctors and other primary care providers of Weston Pelham Park, in the west end of Toronto.


Psychogeriatric Assertive Community Treatment Team

65 years+

More Information

The Psychogeriatric Assertive Community Treatment Team is a multidisciplinary team that serves old adults experiencing serious mental health concerns combined with aging-related complexities.


Seniors Case Management

55 years+ or Adults with Disabilities

The Reconnect Seniors Case Management program helps seniors 55+ and adults with disabilities navigate the health care and social service systems. Our Case Workers will work with clients to plan and coordinate services to help them stay independent and healthy.


Short-term Crisis Bed

16 years+

More Information

The Mental Health and Justice Short-Term Crisis Bed program provides short stay residential support in the community to individuals who are living with serious mental illness, are involved with the judicial system, and are in crisis. This program serves the Greater Toronto Area (GTA).


Streets to Homes

16 years+

More Information

The Streets to Homes program is a partnership with the City of Toronto in which Reconnect offers support to recently housed individuals 16 years of age and over, who have been identified by the City of Toronto as a part of their Housing First Homeless Initiative.


Supporting Clients Currently in Housing

16 years+

More Information

Reconnect provides support to individuals currently living in housing provided by specific organizational partners. In Peel we work with Service and Housing in the Province (SHIP), and in Toronto with the Canadian Mental Health Association (CMHA) Toronto, and Mainstay Housing.


Transitional-Aged Youth Case Management

Youth aged 15-26 years

The Transitional Aged Youth (TAY) Case Management Program provides community-based support for young people living with mental illness and addictions challenges, including those transitioning between the youth and adult mental health systems.


Transportation Service

55 years+ or Adults with Disabilities

More Information

The Transportation program provides door-to-door assisted rides to frail seniors and adults with disabilities. Our program is part of Toronto Ride, a collaborative partnership of thirteen not-for-profit seniors services agencies providing the service.
