Rapid Access Seniors Supports (RASS)


Reconnect and Mount Sinai Hospital have partnered to create a rapid access short-term support service for older adults. 

Program Description

Reconnect and Mount Sinai Hospital have partnered to provide rapid access to seniors facing increased risk due to physical, cognitive, or other age-related health issues. Mount Sinai GEM Nurses will refer patients to Reconnect case managers who will help to stabilize the client’s situation and put in supports to help prevent future crises. The length of stay in this program is 90 days.

Eligibility Criteria

  1. Patient must be 65 years of age or older.
  2. Patient must have a residence.
  3. To be eligible for this service you must be in the catchment area below:
  • South of Eglinton Ave., North of Lake Ontario, West of Bayview Ave., East of the Humber River.

  4. Patient must additionally meet at least 4 of the following criteria:

  • Be assessed as having a score of 6 or higher on the clinical frailty scale.
  • Have experienced acute functional decline in the last three months with no organic cause identified in the emergency room.
  • Have had acute cognitive decline in the last three months affection function and safety at home.
  • Have experienced falls (2-3) in the last three to six months.
  • Have experienced falls and be taking high-risk medications, e.g. benzodiazepines, opiates, sedating or anticholinergic.
  • Be taking six or more medications and experiencing issues around medication compliance.
  • Caregivers experiencing burnout/stress.
  • Be socially isolated.
  • Be experiencing food insecurity.
  • Have experienced a significant (5%) reported weight loss over last few months.
  • Not connected to a family doctor.
  • Experiencing mental health concerns.
