Partner members can click on their link below to refer patients/clients to our services.
West Community Connect makes it easy for health service providers to find mental health, addictions, community support and seniors services for their patients and clients. We work with people who live in the West Sub-Region of Toronto.
Referral for Case Management at Reconnect Community Health Services.
Open to patients of the Sinai-UHN Geri Hub referred by Geriatricians and Clinicians from Sinai Health and UHN Hospitals.
Reconnect and UHN have partnered to provide quick and easy access to seniors services and mental health services in the community for UHN patients.
Reconnect and Mount Sinai have partnered to provide quick and easy access to seniors services and mental health services in the community for patients of Mount Sinai Hospital.
The Primary Care Inter-Professional Team is a partnership of health care organizations supporting the family doctors and other primary care providers of Weston Pelham Park, in the west end of Toronto.
Integrated Transitional Services - Harbour Lights is a TC LHIN initiative created to support the transition from hospital to community for two patient groups: those whose bariatric needs are presenting as barriers to a transition to a community setting, and those whose mental health and addictions needs are presenting as barriers to a community setting.
Mobile Enhanced Assisted Living Services (MEALS) is a community-based support service for older adults and seniors with complex needs who are able to continue living at home, directing their own care (or via a substitute decision maker) as long as they have access to personal supports on a 24/7 basis. The service provides case management, personal support services such as bathing, medication support, incontinence care, light housekeeping and more.
Referrals to Reconnect from CAMH for FACT clients in the community.
Referrals to Reconnect from Humber River Hospital for patients aged 65+.
Short-term case management program focused on system navigation and care coordination for clients who have been recently discharged from hospital and need support to transition back into the community. Referrals are accepted from hospitals, primary care and community agencies.
Reconnect and Mount Sinai Hospital have partnered to create a rapid access short-term support service for older adults.
Reconnect and Humber River Hospital have partnered to provide quick and easy access to seniors services and mental health services in the community for Humber River patients leaving hospital.
