Eligibility Criteria

Mental Health and Addictions Services

Clients Must:

• Be able to live safely in the community without current intensive supports provided by CAMH’s FACT teams
• Be 65+ or 60+ with age-related health issues
• Have current stable housing
• Have a diagnosis of (a) severe and persistent mental health issue(s) 
• Live within Reconnect’s catchment area (Islington Ave to Yonge St, Eglinton Ave to Lake Ontario)

Please Note: Individuals posing a risk to OTRs will be assessed for admission on a case-by-case basis. 

Services for Seniors

Clients who also need additional seniors supports must:

• Be 65 years or older or who are 60+ with age-related complex health issues. 
• Be able to live safely in the community without current intensive supports provided by CAMH’s FACT teams
• Have a discharge destination
• Have all equipment necessary in the home to live there safely
• Test negative for Covid-19 prior to admission
• Be able to feed themselves 
• Be able to take their own medication with supervision 
• Be able to self-direct in and out catheterization (if required)
• Live within Reconnect’s catchment area (Islington Ave to Yonge St, Eglinton Ave to Lake Ontario)
  

Please Note: 

• Clients who are on LTC and LTC Crisis waitlists are eligible for service. 
• Clients requiring 8+ hours of care a day are NOT eligible.
