William Osler Health System Partnership

Reconnect and William Osler Health System has partnered to provide quick and easy access to our Central West Concurrent Disorders Specialized Support Services for patients at the Etobicoke General Hospital.

Eligibility Criteria

  • The patient must be 16+
  • The patient should be inpatient in hospital or a long-standing hospital outpatient of Etobicoke General Hospital.
  • The patient must present with both mental illness and addiction (concurrent disorders) 
  • The patient must live in Etobicoke Rexdale or Dufferin County, including Malton, Brampton, Orangeville and Shelborne.
  • The patient must have a residence.
