George Hull Reconnect Transitional Aged Youth Partnership
Reconnect Health Services and the George Hull Centre for Children and Families have partnered to support youth transitioning out of child/youth mental health services into adult services.
Eligibility Criteria
- Youth aged 15+
- Youth must be enrolled in George Hull Centre for Children and Families’ Programs
- Youth who are living with mental health and addiction challenges
- Youth must be willing to receive services from Reconnect Health Services
- Youth must live within the Reconnect catchment area (Yonge St to Highway 427, and Eglinton to Lake Ontario) or be willing to come to a Reconnect office for all visits.
