Eligibility Criteria

Services for Seniors:

• The client must be 55+
• Clients must have a residence.
• The client must live within Reconnect’s catchment area for the desired service. This is roughly the area west of Yonge St ,south of Lawrence Ave, north of Lake Ontario and East of Humber River. Please see program descriptions for further program-specific eligibility criteria.

Mental Health and Addictions Services:

• The client must be at least 15 years of age.
• Clients must have a residence.
• The client must present with persistent and complex mental health issues. Serious mental health issues may be defined by the duration of the illness and the level of disability it has caused. Primary diagnoses may be schizophrenia, schizo-affective disorder, bi-polar disorder and/or chronic depression. Clients who have a secondary diagnosis such as mild developmental disability, brain injury, and/or substance use problems and problems regulating emotions are eligible. Clients with only addictions issues are not eligible.
• The client requires case management for unmet community health care needs. This includes system navigation and support to stabilize mental health symptoms.
• The client and/or their substitute decision maker are willing to accept services.
• The client must live within Reconnect’s MHA services catchment area, i.e. generally west of Yonge St, south of Eglinton, north of Lake Ontario and east of Highway 427, although some programs catchment areas will differ.
