UHN Seniors Emergency Medicine Centre Partnership
University Health Network and Reconnect Health Services are partnering to provide rapid access to Seniors Services and Mental Health Services in the community for older adults presenting to Toronto General and Toronto Western Seniors Emergency Departments.
Eligibility Criteria
Patients must:
- Be aged 55+
- Have a residence where they can be found and contacted. If someone is transient or experiencing homelessness, they can access Reconnect support through our Sound Times program at 280 Parliament Street, Toronto, Ontario M5A 3A4, Canada.
- Be willing to receive services from Reconnect Health Services.
- Be able to be safely supported in the community.
- Be in the catchment area: south of Eglinton Ave, north of Lake Ontario, west of Bayview Ave and east of the Humber River.
