Case Management

16 years+

Case management offers one-to-one supports by a case manager who will help you to identify and work towards your healthcare goals. Case managers will assist you in developing strategies to maintain self-care, symptom management, crisis management, and developing a support network, as well as life skills including budgeting and grocery shopping.

Eligibility Criteria: To be eligible for service, an individual must:

• be at least 15 years of age
• be experiencing a serious mental health issue or be someone who's mental health is impacting their ability to manage their healthcare needs
• have goals they want to work towards

This program serves those living in the area east of Highway 427, west of Yonge St., south of Eglinton Ave., and north of Lake Ontario.
