Mental Health and Justice Prevention Case Management

The Mental Health and Justice Prevention Program provides case management to people 16+ who struggle with mental illness, addictions, and/or cognitive disabilities, and who are currently involved in the criminal justice system (or are at a high risk of becoming involved). Further eligibility requirements can be found on the referral form. The Prevention program serves the Central Toronto Area and North Etobicoke. The catchment area for the Prevention program is roughly as follows: east of Islington Ave., south of Eglinton Ave. W. and Eglinton Ave E., south of York Mills Road, west of Wilket Creek, west of Warden Ave., and north of Lake Ontario.
