Mobility+

Eligibility Criteria

• The patient must be inpatient with Toronto Rehab.
• The patient must be medically stable and cleared for hospital to home transition.
• The patient must be willing and able to participate in services.
• The patient’s home must be safe and conducive to providing and receiving services.
• The patient must have enough mobility to sit and stand on their own or with the assistance of one person, and to be able to answer the door to allow staff to come into the home.
• The patient must have a physiotherapist-created rehabilitation program ready for implementation.
• The patient must live in the Reconnect catchment area: Humber River to Bayview Ave and Lawrence Ave to Lake Ontario. Patients being discharged to a Transitional Care Unit (TCU) within that catchment area are eligible for services.  

Exclusionary Criteria

• Patients with unstable vitals
• Patients with unaddressed fall risks
• Patients with acute pain that significantly limits them, so they don’t meet the above criteria, and prevents participation in exercise
• Patients who won’t take direction
• Patients with behavioural issues that are not well managed, and might affect participation
• Bedbound patients
• Patients with bed bugs
• Patients requiring a two-person assist
