Shift:

Monday to Friday 1:00pm to 8:30pm

Sunday 8:00am to 8:00pm

 

Summary:

The Personal Support Worker provides personal care, homemaking and related duties to Community, Supportive Housing and Transitional Care Unit clients.

 

Responsibilities include, but are not limited to, the following:

  • Assists clients in their units with personal care including bathing, dressing, toileting, incontinence care, ambulation and client repositioning. Uses transfer and lift techniques and/or equipment e.g. Hoyer Lift.
  • Performs Light homemaking and other related duties in clients’ units, such as meal preparation, washing dishes, vacuuming, mopping floors, routine cleaning (toilet, tub, sinks etc), and laundry. Uses mops, vacuum cleaners and other routine household appliances.
  • Encourages social engagement with clients by facilitating conversations, encouraging clients to part take in rehabilitation exercises and facilitating outings to reduce isolation and promote client’s mental well-being. 
  • Assists with medication as follows:
  • Provides medication reminders to clients
  • Provides verbal prompting to a client who are required to take medication
  • Hands medication to a client from a preset daily blister pack
  • Reports any noted irregularities such as medications not taken, medications taken too often or medications being taken at the wrong time to their Manager
  • Documents the completion of medication related tasks in accordance with the organization’s Medication Management Policy
  • Travels and provide service to clients in the community, within Supportive Housing Buildings and Transitional Care Units.
  • Reports and documents clients’ behavioral changes, physical hazards, safety, task changes or other sources of concern to Manager.
  • In the event of a client emergency, including when a client seems or indicates they are unwell, calls 9-1-1 and then reports situation to a manager
  • Attend staff meetings as required;
  • Follows all health and safety guidelines established by Reconnect’s Joint Health & Safety Committee;
  • Fulfills other responsibilities as required, possibly for clients of other Reconnect programs.

 

Technical and Behavioral Competencies Required:

  • Post-secondary Personal Support Worker Certificate, completed with an accredited educational institution.
  • Proven experience working in a group living, community or long-term care setting for one year or more
  • Good written and verbal language skills
  • Knowledge and experience working with seniors, frail elderly, chronically ill, cognitively impaired, and/or disabled individuals
  • First Aid/CPR Certification an asset
  • Good problem-solving skills,
  • knowledge of and sensitivity to gender, race, sexual orientation, age and multicultural issues
  • Interacts with clients, family members, staff, visitors and other care providers

 

Working Conditions

  • May be exposed to infectious waste, diseases, conditions, etc., including exposure to the AIDS and hepatitis B and C viruses.
  • May be exposed to environments that have pests, infestations, hoarding, lingering cigarette smoke

 

NOTE: Reconnect requires a criminal background check for all employees, volunteers and students.

 

Workplace Requirements:

  • Generally employees work a 35 hour work week.
