The Psychogeriatric Assertive Community Treatment Team is a multidisciplinary team that serves individuals experiencing serious mental health concerns and comorbidity combined with aging-related complexities like early onsite of Dementia. The service includes social work, nursing, occupational therapy, behaviour therapy, geriatrics and geriatric psychiatry.


Eligibility Criteria: To be eligible an individual must be 60 years or over (with some exceptions for those 55+), living between Eglinton Ave/401 to Lake Ontario, priority will be given to those living west of Yonge St, east of Kipling, south of Eglinton to the Lakeshore.


The individual must be experiencing serious mental health problems, be having difficulty living in the community, have an inability to consistently manage their daily living activities or to maintain a safe living situation, and have had multiple recent hospitalizations or use of hospital emergency departments and require medication management.
