Psychogeriatric Assertive Community Treatment Team

The Psychogeriatric Assertive Community Treatment Team is a multidisciplinary team that serves older adults experiencing serious mental health concerns, combined with other physical and/or cognitive health problems associated with aging. The service includes social work, nursing, occupational therapy, behavioural therapy, geriatrics, and geriatric psychiatry.


Eligibility Criteria: To be eligible an individual must be 60+ (with some exceptions for those 55+), living between Eglinton Ave/401 to Lake Ontario. Priority will be given to those living west of Yonge St, east of Kipling, south of Eglinton to the Lakeshore.


The individual must be experiencing serious mental health problems and having difficulty living in the community. Eligible participants are unable to manage their daily activities and maintain a safe living situation. They must also require medication management and have had multiple recent hospitalizations or emergency hospital visits.
