If you'd like to refer yourself or someone else to this service, please call 416-248-2050 x 8038 or use the red button below to book a phone appointment with our Intake Department. 

The Reconnect Case Management program helps seniors 55+ and adults with disabilities navigate the health care and social service systems. Our Case Workers will work with you to plan and coordinate services to help you stay independent and healthy in your home. This can include:


  • Help arranging for health services, home supports, transportation or Meals on Wheels
  • Assistance with filling in forms and completing applications including rental applications, filing your taxes, and completing CPP or other income support application forms.
  • Support group referrals, education and training for family and caregivers. 


To be eligible for service, you must live within our service area. Please see the map below. Services can be provided in French and English.
